Cheif    complaint A 64 year old male patient came to medical OPD with chief complaints of pain in right loin since 1 month and vomiting since 1 month and burning micturition Since 1 month . Here we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those patient's clinical problems with collective current best evidence based inputs. # This  is an ongoing case , and will be updated as and when the information is provided CASE SCENARIO A 60 yrs old male patient was Auto driver by occupation came to OPD with the C/o constipation since 2 months pedal oedema from 15 days SOB for 15 days  and cough for 7days  HISTORY OF PRESENT ILLNESS  Pt was apparently asymptomatic 6 yrs back and had c/o constipation and blood in stools and underwent medication for 1 month and haemorroids subsided  4 yrs back pt had sever vomitings and giddiness which  diagnosed with DM and HTN and prescribed medicines and patient was on medication since then  Now since 15

CASE SCENARIO  Pt aged 75 yrs M farmer by occupation came to opd with  c/o 5 days back(Tuesday) has loose stools(4 episodes) and fever for 2 days (tue and wed) then on Friday he went to a pvt hsptl with c/o of generalised weakness  . HISTORY OF PRESENT ILLNESS Pt was apparently normal 6 yrs back and got diagnosed with  pulmonary TB  and took treatment for 1 yr and got controlled on TB. Then he again started smoking and 3 yrs back he again again developed wheeze and on medication for TB                3 months Back the wheeze is aggregated and and 5 days back he has 4 episodes of loose stools which were non mucous non blood stained  watery and small quantity and fever for 2 days and generalised weakness since 5 days and  grade III dyspnea  since a week HISTORY OF PAST ILLNESS   Pulmonary TB and on medication  N/k/c/o DM,HTN TREATMENT HISTORY Pt is on TB medication .
